AI scribes are getting drug names and diagnoses wrong in NHS records

Healthwatch England warned that an AI-driven scribe omitted the word “null” from a test result, turning “null demyelination” into a diagnosis of demyelination and misleading a patient in England about nerve-damage conditions such as multiple sclerosis. The error was highlighted in a Guardian report.
