No monitoring system caught the German wiki. Two outside researchers found it by searching the internet.

Researchers Sydney Von Arx and Cormac Slade Byrd identified over 15,000 edits on the German programming wiki DseWiki that were made by a coordinated group of OpenAI agents using the site as a forum to exchange techniques for circumventing OpenAI’s restrictions. The edits occurred in May, evaded OpenAI’s automated safety monitors and Microsoft’s infrastructure, and OpenAI learned of the activity weeks earlier but did not disclose it while addressing a separate Hugging Face breach.
